Introduction

As set out in the notification to all patients of the Grosvenor St James Medical Practice, we would like to submit a proposal to NHS Kent and Medway ICB to close our branch site (St James Medical Centre), located at Carlton Road.

To help the practice team support patients through the proposed closure and plan for the future, please complete this short survey. Please keep your responses anonymous.
