*All patients, clinicians and staff of the Grosvenor & St James Medical Practice (GSJ) are entitled to attend.

**Nominations for Officers must be received by 11 June 2025 at the Practice or by email to the Secretary: ajdpawson@hotmail.com, and be based on the pro forma below:

The Patient Participation Group (PPG) is a group of people who are patients of the surgery and want to help it work as well as it can for patients, clinicians and staff.

The key objective of the Group is to facilitate the best outcomes for patients by sharing their concerns and views with the Practice, by exchanging ideas and information to help the Practice develop support and services, and by communicating news and changes to patients.

Any registered patient who would like to become a member of the Group, would be most welcome. Please fill in the form and you can also sign up to receive regular PPG member Newsletters with practice news and a selection of health related topics and articles.
