What is Health and Wellness Referral Programme/Scheme?

The Health and Wellness Referral Program (HWRP) at Tunbridge Wells is designed for individuals over 18 years old, offering tailored support for those who face health risks and/or lead a sedentary lifestyle.

Through personalised guidance, participants will be able to enhance physical, mental, and social well-being under professional supervision. This structured program prioritises integrating exercise into weekly routines and is particularly beneficial for individuals with specific medical conditions.
The process begins with a referral from a GP, health professional, or self-referral. Our dedicated Health and Wellness team then will design a safe and personalised exercise program, providing continuous support throughout a six week journey. How to get a referral

To kick-start your journey to a better and improved health, simply secure a referral. You have the option to either self-refer by completing the referral form available here or you can undergo screening and get referred by a trusted health professional.  

Referred patients will enjoy a thorough 6-week programme with the opportunity to participate in two classes each week for a total of 12 sessions. Additionally, specialist classes will be available dependently on a health condition of a referred person. All our instructors hold Level 3 Exercise on Referral qualifications. Participants will be empowered to set personal goals, supported in enhancing dietary habits, and encouraged to engage in regular exercise.

What is the principle aim of the scheme?

The principle aim of the scheme is to support, empower and motivate participants to make positive lifestyle changes through regular physical activity and healthy eating. By working closely with qualified instructors and
healthcare professionals, participants can safely and effectively improve their overall health and quality of life.

 

What are the benefits of regular exercise?

Eligibility criteria

Local residents aged over 18 years old with undergoing health condition/sedentary lifestyle.

Or, one or more of the following conditions:

  • Obesity
  • Hypertension
  • Hyperlipidaemia
  • Anxiety
  • Diabetes
  • Depression
  • Severe mental health
  • Musculoskeletal
 

Exclusion to the scheme:

  • Over 65 and at risk of falls
  • Unstable or uncontrolled cardiac disease or recent cardiac event
  • Heart failure
  • Angina
  • Uncontrolled BP over 180/100
  • Resting tachycardia >100 bpm
  • Claudication
  • Recent stroke/TIA (unless referred by neuro-rehabilitation)
  • Severe Osteoarthritis
  • Dizziness or Syncope
  • Orthopnoea or PND
  • Severe or brittle asthma COPD
  • Poorly controlled diabetes
  • Any medical condition not controlled
  • Any musculoskeletal conditions that do not allow independent exercise
