Register your Type 1 Opt-out preference
If you don’t want your identifiable patient data to be shared for purposes except for your own care, you can opt-out by registering a Type 1 Opt-out.
This collection will start on 1 July 2021 so if you do not want your data to be shared with NHS Digital please register your Type 1 Opt-out with the surgery by 23 June 2021.
The data held in your GP medical records is shared with other healthcare professionals for the purposes of your individual care. It is also shared with other organisations to support health and care planning and research. If you do not want your personally identifiable patient data to be shared outside of your GP practice for purposes except your own care, you can register an opt-out with your GP practice. This is known as a Type 1 Opt-out.

To register a Type 1 Opt-out, for yourself or for a dependent (if you are the parent or legal guardian of the patient) (to Opt-out) you can fill the form from the link below before the deadline of June 23rd 2021**. After this date, the type 1 opt out would still be respected, however, this would be on new data extracted post the deadline of 24th June 2021 and not over data that has already been extracted.

** Please note that the GPs and British Medical Association are lobbying for delay in roll-out of patient data sharing programme beyond the June 23rd 2021 deadline.
It is important to note that while the type 1 opt-out will apply to some data sharing, it won’t apply where there is an overriding legal requirement for GP practices to share this data with NHS Digital. A type 2 opt out (now called National Opt Out) prevented information being shared outside NHS Digital for purposes beyond the individual's direct care.
National Opt Out: Patients can choose to opt out from the use of their data for research or planning purposes. Confidential (identifiable) data held by NHS digital cannot be shared for research and planning once this opt-out is completed. Patients must complete this themselves as the Practice is unable to register the patients decision to ‘opt out of this data sharing’. Please visit NHS Digital to make informed choice.
